If you have not already scheduled your appointment please do so by calling (512)306-8360. Please have your personal and insurance information ready – as we will need it in order to verify your coverage prior to your appointment. Self-pay patients are welcome at our office – please see our Payment Policy Form. After setting your appointment continue on to the instructions and resources below.
If you have already set your appointment click the links below in order to complete and print out your new patient forms – please bring your completed forms with you to your appointment. You will also need to bring your photo id and insurance card to your appointment as well.

- HIPAA Records Release Form - Please complete this form if have healthcare records to obtain from another provider. Bringing those records with you to your appointment will be very helpful in beginning your healthcare with Capital Family Practice.

- Patient Portal Guidlines Form - If corresponding with your personal healthcare provider via a secure online portal is something you would like please complete this form. The Patient Portal is a convenient and secure way to request prescription refills, receive blood test results, have questions regarding your personal healthcare answered, and more.

Scheduling: Please call for scheduling variations, as all of our providers do not work the same hours and each provider has specific scheduling preferences.
Mental Health: If being seen for a mental health issue please check with your insurance carrier to assure that it is covered under your plan. If not you will be responsible for all applicable charges. Also keep in mind that our providers have different scheduling preferences and availability when it comes to mental health appointments.
Please be at least 20 minutes early to your appointment as there may be additional paperwork for you to complete.
