In an effort to keep all of your information current within our systems please complete our Patient Information Form if you have not already done so, have not been seen in a while, or have made changes to your personal information.
Scheduling: Please call for scheduling variations, as all of our providers do not work the same hours and each provider has specific scheduling preferences.
Appointment Cancellations: If you are going to cancel your appointment please do so 24 hours prior to your appointment. For physical appointments that are not cancelled 24 hours prior to your appointment there may be a cancellation fee charged to your account.
Insurance: If you have made changes to your insurance coverage please call our office and provide us with your new insurance information. This way we have your information verified before your next appointment. If changing insurance carriers please refer to our Insurance Page to see which insurances we are currently accepting.
Mental Health: If being seen for a mental health issue please check with your insurance carrier to assure that it is covered under your plan. If not you will be responsible for all applicable charges. Also keep in mind that our providers have different scheduling preferences and availability when it comes to mental health appointments.
Patient Portal: Several of our physicians are now utilizing electronic medical records. This provides more access and flexibility for both patient and doctor, due to a direct intergration with patients’ “electronic charts. Both you and your doctor will have instant, 24-hour online access to your up to date medical information via the “Patient Portal.” This portal functions much like your bank’s secure online website, and utilizes individual usernames and passwords via a secure website to allow you to access your private medical information.
