Medical Records and Forms
 
For completeness, our doctors prefer to have any medical records available transferred to our office.  A form to request to transfer your medical records to our clinic is available on the Home Page under Patient Education Resources and is called the Medical Records Release Form.  To send your medical records from our clinic to another doctor, we need a written request from you and the appropriate fee which is usually $25.  Also, for detailed forms such as a FMLA form, a physical form, etc. not done at an office visit, there will be a $25 fee.

 

Travel Clinic
 
For international travelers, Capital Family Practice provides required and recommended immunizations as well as advice on avoiding local illnesses, managing “jet lag”, and dealing with local pests and other health threats.  CFP researches the specific area of interest down to the city or area, obtaining the most up-to-date information to maximize a successful trip for you and your family.  Please make an appointment by calling 306-8360 at least 4-6 weeks prior to your departure date and give us your specific itinerary and dates of travel.  Insurances do not routinely cover travel consults and immunizations, so you will be responsible for those costs at the time of your visit.

 

Prescription Refills and Preauthorizations
 
Prescriptions are typically given at office visits with enough refills until your next visit.  In the event a refill is needed sooner, you or your pharmacist may call us to request a refill.  We do not accept fax requests.  A $10 convenience fee will be added to your account if it is approved instead of requiring an additional office visit.  “Triplicate” prescriptions also require a $10 fee and must be picked up at the office. We will not mail or call in triplicate prescriptions.  If your insurance company requires a preauthorization for your medication, you have three options:
1) Discuss other options with your pharmacist that do not require a preauthorization and the pharmacist can call us to request a change.
2) Make an appointment with your doctor so he or she can block off that time to call your insurance to try to obtain approval. This office visit will require a copay.
3) You may pay a $25 administrative fee to have your doctor call or fill out paperwork to try to obtain authorization.

 

After Hours Care
 
For a true emergency, call 911 or go to the closest Emergency Room.  After the clinic is closed, a physician or physician assistant is always on call for problems that cannot wait until the clinic opens.  Emergency or urgent calls are routed through the Travis County Medical Exchange at 458-1121.  Be sure to turn off your “call blocking” feature if it is on and have a number of a pharmacy that is open.  For after hours calls and consultations, a $25 fee may be assessed to your account.

 

Payment Policy
 
We attempt to verify the insurance status of every patient before seeing the doctor or physician assistant.  If we cannot verify your insurance, you will be responsible for the total charges at that visit. We will issue a prompt refund after the insurance is verified and claim is processed.  All fees, deductibles, past charges, or copays are due at the time of your visit.  We will file your insurance claim for you and will notify you of any outstanding charges when your claim is processed.  If you cannot afford your visit, please notify our billing office or receptionist PRIOR to your visit so arrangements can be made.  We accept cash, local checks, Mastercard, Visa, and Discover cards. Vaccine Fee List
 
Most insurances pay for REQUIRED childhood immunizations, but most insurances do not pay for RECOMMENDED shots or travel vaccines.  Below is a list of vaccines offered for your convenience at competitive prices in our clinic:

FluMist Nasal Spray Vaccine
 
FluMist is a live, weakened flu vaccine that is a nasal spray not a shot and costs $35.  If you answer NO to the following questions, you are probably eligible to receive the FluMist vaccine:

*Are you 23 months old or younger?
*Are you 50 years old or older?
*Do you have asthma or reactive airway disease?
*Are you pregnant or breast feeding?
*Are you on chemotherapy or have a lowered immune system like AIDS?
*Are you taking care of someone who is on chemotherapy or has a lowered immune system?
*Do you have Diabetes?
*Do you have kidney function problems?
*Do you have heart disease like Heart Failure or lung disease like emphysema?
*Are you allergic to eggs?
*Are you between 2 and 17 years old and take aspirin every day?
*Do you have Guillan-Barre?

If you had a vaccination or anti-viral medication within the last month, please call our office and speak with one of our medical staff to see when you will be eligible for the FluMist.  If the patient getting this vaccine is between 2 and 8 years old and is getting FluMist for the first time, they will need a booster dose in one month which is $35 too.  If you are not a candidate for the FluMist or would rather receive the flu shot, you can drop in without an appointment Mon.-Fri. from 8-11am and 2-4pm.  This service is for new and established patients and is $25.  If you have any questions, please call our office at 306-8360.

 

Research Opportunities
 
Capital Family Practice is working with MetaClin Research to offer our patients research opportunities.  Most research studies offer free lab and other studies, free medication, and a stipend for participating.  MetaClin Research, Inc. offers Phase I-IV multi-specialty research services focused on patient population trials.  MetaClin has a talented research staff and innovative facilities, both outpatient and inpatient, specifically designed for medical research.  They are well known in the central Texas area for our high quality and our close relationship with the community.  These relationships include those with medical providers with expertise in multiple therapeutic areas and with diagnostic testing centers which may provide specialized tests and procedures during the course of a research study.  These relationships also include our subject volunteers; we pride ourselves on focusing on the health, well being, and quality of each trial participant.
